Houston, TX (CompNewsNetwork) - Private providers of TX vocational rehabilitation services may apply to be listed in the Texas Department of Insurance - Division of Workers' Compensation Registry of Private Providers of Vocational Rehabilitation Services, provided below.
Rule 136.2 http://www.tdi.state.tx.us/wc/rules/136.pdf requires a private provider who wishes to be included in the Division's registry to complete, sign and submit an agency-approved registration form. Rule 136.2(b)(5) requires a statement showing that the private provider is credentialed as an LPC, CCM, CRC, CVE or CDMS.
Although only individual private providers can be included in the registry, an individual provider may list a business affiliation if desired. The application is valid for a one-year period.
After reviewing the application and attachments to determine if the provider meets the qualifications set out in Rule 136.2, TDI-DWC will send the provider written results of the review.
