Austin, TX (CompNewsNetwork) - The Texas Department of Insurance, Division of Workers' Compensation (TDI-DWC) has proposed revisions to the DWC Form-069, Report of Medical Evaluation. Doctors selected by TDI-DWC, an insurance carrier or the treating doctor must use this form when evaluating permanent impairment or maximum medical improvement of an injured employee. In addition, treating doctors may agree or disagree with the other doctors' findings on this form. The proposed form is available for download from the TDI-DWC website at http://www.tdi.state.tx.us/wc/forms/documents/dwc069draft.pdf. TDI-DWC is accepting public comments regarding the proposed revisions to DWC Form-069 until 5:00 p.m., August 16, 2010.

The purpose of this memo is to request input from system participants regarding the proposed changes to DWC Form-069. Since this is not a formal proposal, public comments received will not be responded to in accordance with the Texas Administrative Procedures Act.
The proposed revisions to DWC Form-069 are intended to provide more clarity and harmony with 28 TAC §130.1 regarding Certification of Maximum Medical Improvement and Evaluation of Permanent Impairment.
