Two NY Doctors Removed From WC Treatment List

New York, NY (CompNewsNetwork) - Please be advised that, effective May 26, 2009, Hershel Samuels, M.D., voluntarily resigned his authorization to treat and conduct independent medical examinations of workers' compensation claimants.

Dr. Samuels is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Samuels for treatment rendered or examinations conducted prior to May 26, 2009, are valid, but are invalid for any treatment rendered or examinations conducted on or after that date. Requests to cross-examine Dr. Samuels regarding treatment he rendered or examinations he conducted prior to May 26, 2009, should not be denied due to this resignation.

Please be advised that, by Determination and Order (BPMC No. 09-62), effective April 14, 2009, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), sustained the charge of Craig Blum, M.D., having been found guilty of driving while intoxicated.

Pursuant to Workers' Compensation Law Section 13-d [2][a] and [5], the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Blum from the list of authorized physicians, effective May 26, 2009. Dr. Blum's last known address is S-3673 Southwestern Boulevard, Orchard Park, New York 14127.

Dr. Blum is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Blum for treatment rendered prior to May 26, 2009, are valid, but are invalid for any treatment rendered or examinations performed on or after that date. Requests to cross-examine Dr. Blum regarding treatment he rendered or examinations he conducted prior to May 26, 2009, should not be denied due to his removal from the list of authorized physicians.
