TDI-DWC Centralizes Processing Of DWC Form-069 And Narratives

                               Dallas, TX (CompNewsNetwork) - No later than March 1, 2011, workers' compensation system participants must fax all completed DWC Form-069, Report of Medical Evaluation and associated narrative attachments to the Texas Department of Insurance, Division of Workers' Compensation (TDI-DWC) at 512-490-1047. Do not send these reports and narratives to the TDI-DWC Field Offices or other agency fax numbers. System participants may begin using this fax number at any time.
 
On and after March 1, 2011, the TDI-DWC is also requiring all system participants to use the new DWC Form-069. The completed forms must be submitted to the TDI-DWC at the fax number listed above.
