NY's DWC Post Doctor's Narrative Report Online

                               
New York, NY (CompNewsNetwork) - The Doctor's Narrative Report (EC-4NARR) is a new form the Board is providing to health care providers to assist with filing medical reports with the NYS Workers' Compensation Board.

The EC-4NARR form will only be accepted by the Board through electronic submission. Every EC-4NARR filed with the Board is electronically signed by a Board authorized health care provider in compliance with the New York State Electronic Signatures and Records Act (ESRA) and its accompanying regulation (9 NYCRR 540).

Instead of filing the Doctor's Initial Report (C-4) and Doctor's Progress Report (C-4.2) forms, health care providers may file the Doctor's Narrative Report (EC-4NARR) to report both the initial and continued treatment, however, a narrative attachment must be included with the form which provides all the additional information which otherwise would be provided on the other forms. To report permanent impairment, submit form Doctor's Report of MMI/Permanent Impairment (C-4.3).

Over the years, the Board has had many requests from health care providers to be able to save doctor/patient/carrier information to be used to submit subsequent progress reports to the Board. The new web submission form will provide this desired feature.

Civil Service Employees Insurance Group Promotes Schulhofer To VP Of Sales

San Francisco, CA (CompNewsNetwork) - Civil Service Employees Insurance Group has promoted Kelli L. Schulhofer to vice president of sales from regional marketing director. In her new role, Schulhofer will be responsible for CSE's overall agency field force and the company's distribution network in Arizona, California and Nevada.

Schulhofer joined CSE Insurance in 1998 as a senior territory marketing representative for the Los Angeles, San Bernardino, Ventura, Santa Barbara, and Bakersfield, Calif., areas. She was promoted to regional marketing manager for California in 2005, and named regional marketing director for California in 2007. During her 10 years with CSE, she has supervised marketing representatives in California, Arizona, and Nevada, and has managed advertising co-op programs and strategic marketing opportunities on behalf of CSE agents.

Prior to CSE, Schulhofer was vice president of insurance operations for Coast Fed Services in West Hills, Calif., from 1995 to 1998. She also served as an underwriter for Auto Insurance Specialists in Woodland Hills, Calif., from 1990 to 1995, and an office manager for Bohannan & Associates, Farmers Insurance Agency, in Van Nuys, Calif., from 1986 to 1990.
