NC's Commission Posts Revisions To Mandatory Form

Raleigh, NC (CompNewsNetwork) – The N.C. Industrial Commission has revised Form 17, which is now titled the N.C. Workers' Compensation Notice to Injured Workers and Employers. The new form can be found at http://www.nc.ic.gov (PDF file). Effective February 1, 2009, this form must be prominently posted, if you have workers' compensation insurance or qualify as self-insured. (N.C. Gen. Stat. §97-93). It must be printed in the same colors and format that appears on the Industrial Commission web site.

The NCIC will exhaust all available I.C. file numbers within the next month. A new file numbering system will be implemented when this occurs. The new file numbers will begin with a “W” followed by five digits. For example, “W13276.” A notice will be posted at, http://www.ic.nc.gov/, when the switch occurs.
