Long story short, my case is in final stage. My lawyer asked me to tell my pain management doctor to fill out the work restriction form and he refused to fill out, he said he is not treating me because I don’t want to take the injection that he suggested, these years he only gave me the pain killer and he’s my only doctor. Also my lawyer will send him the request for MMI exam, most likely he will send me to the orthopedic doctor, do you think the orthopedic dr will fill out the form for me? If not, should I look for a new dr in this stage?

A good atty will let the doctor know before the exam what information is needed. I don’t know your state so can’t say f you are allowed to change doctors.

If you are going to need additional care for your injury, it should be with a doctor you trust.

If your case is so important, why are you staying with a doc that is Uncooperative?
