if there has been a homicide contact your local police department.
for deliberate falsehoods by your doctor file a fraud complaint with your local district attorney.
also file a complaint with your states Medical Licensing board.

you have 12 weeks of job protection from FMLA; file a complaint.
the employer is not required to provide light work.
workers comp benefits are not based on having to prove negligence.

also contact your state insurance department and file a fraud complaint:

are you in a union?
do you vote republican?
did you contact your local legislative representative?
do you support worker issues on your ballot and on your candidate choices?

I'm sorry you continued to work in a knowingly unsafe and hazardous work place and eventually became injured.
it often takes a serious injury or a number of injuries for employees to acknowledge the problem and do something to protect themselves and fellow workers.

worker rights and protections are continuing to deteriorate across the country at an alarming pace.
it's too bad more isn't done by workers and for workers before injuries happen.

it looks like there could be some long term anger and depression.
this is a common set of symptoms when dealing with serious injuries and constant pain.
if left untreated it can become chronic and more debilitating then the original accident.
talk to your doctor and ask for a referral for counseling.
even one session can help determine if there is a problem that can be treated.
it doesn't hurt to find out and could open the door to some relief.
