while I was at work Thursday I bent over and sent my back into horrible spasms, with pain shooting down my leg. As of today ( Saturday) I am all but unable to walk.
I live and work in New York State. I just don't know if I should file a report.

yes, file a report. let the doctors decide as to the cause.
pain itself is not an injury but you need to find out what's causing the pain and whether its from your job duties.

Absolutely file an incident report with your employer than file your WC claim. If you have an up and up employer they should make a call for you to give the details of the onset of pain to the claims adjuster for the ins o to make a determination. Much has to do with your job duties and if you do alot of repetitive motion using your back. Once all the fact are in Im sure your determination will be in as well. just remember you dont have to agree with the determination and can ask the state for an ime
good luck
