1171: I contacted the I&A officer requesting a form to dismiss my case. He said the only forms they have for me is the document separator sheet and proof of service and for me to write a letter to the Judge at WCAB requesting dismissal of case, and I need to arrange the paperwork in the proper order.
My question is do I send just a copy of the letter to all the parties involved or do I also send a paper showing who received the letter like a proof of service along with a copy of the letter to each individual. In the packet I have to make for the court do I list each party that I am sending a copy of the letter to on the same proof of service form or does it need to be individual forms? Not sure how to prepare everything. Thank you.
