I'll guess the risk of a small fine for not sending the proper notice is much less then the cost of sending the notice.
Did you see the report the doctor sent to the carrier?

I have not seen the notice that the doc has sent. So how do I go about retrieving that letter?

Call your Dr office and ask for a copy... I ask for copies of all my reports ( when I can ) another member on here told me to always grab ahold of them and hold on to them for future reference.

Ok, now that my ttd checks have stopped and I am released to go back to work now after there months, does this mean that my doctor gave me an MMI? And if so, what is next?

no, workers can often return to work even though recovery and healing is still taking place.

your vision is impaired and the doctor wants to monitor your progress.
you should be asking more questions of your doctor and your atty.
write them down as they occur to you before a call or visit so you don't forget them.
