Yes. But they only sent the first page. They downplayed it and said it wasn’t clear so I’m not sure where to go after that. I should be receiving the qme report soon so hopefully I can use that report to find an attorney.

This is a notice saying you qualify for 6 grand for school to help retrain you. $1,000. of that can be used towards purchase of computer or computer equipment. I think they reimburse once you have purchased and also once you pay for retraining, not sure. By receiving that notification it also means you may qualify for a check for 5 grand from an additional fund for the RTWSF. You can apply online in this link or take that notice down to a district WCAB office (not regular WCAB) and go to a kiosk and apply there. This check can be used for anything you want. Do you want a check for 5 grand? If so please listen to me and apply and if you are overwhelmed I can walk you through it. I know you are short on money right now and I would like you to apply for this to see if you can get it. There are no guarantees but I would like you to try as it is a simple form to fill out. Let me know if you need my help. Again you can also take that form to an I&A officer and ask them to help you apply for RTWSP. They may reply that the form you have is not for that, which is correct, but you need that form to qualify for the second benefit.

Eligibility for RTWSP payment:

Date of injury must be on or after January 1, 2013.
Applicant must have received a SJDB voucher for that injury.
Apply by the deadline. RTWSP must receive your application within one year from the date you were mailed the SJDB voucher

RTWSP is a check for 5 thousand dollars that you can use for anything. Please apply.

I double check just to be sure while waiting for I&A to return my call. I just noticed that the P&S box isn’t checked. Do I need to be at P&S before I apply?

That form you received states you need to have received that voucher for SJDV to apply. You received it. It says:

"Eligibility for RTWSP payment:

Date of injury must be on or after January 1, 2013.
Applicant must have received a SJDB voucher for that injury."


You received a SJDB for your injury AND your date of injury is after 1/1/2013.

Again, the notice you received is for school/retraining called SJDV. Once you receive that voucher (which you did) means you may qualify for a second benefit called RTWSP. The RTWSP is for California injured workers who's employers will not allow them to return to work. It is a fund set up. I have no idea how long it takes to get it. You can ask an I&a officer if you go to a kiosk if you get it sooner or right away. Make sure you tell them you want to apply for RTWSP. That is the check for 5 grand. We can talk about the other benefit after this. I just want you to apply.

Again if you are overwhelmed let me know and I will help walk you through it. OK?

Return-to-Work Supplement Programen Español
Employees injured on or after January 1, 2013, who have received a supplemental job displacement benefit voucher (SJDB), may also qualify for an additonal one-time $5,000 payment.

The Return-to-Work Supplement Program (RTWSP) will mail approved applicants a $5,000 check that can be used to supplement the earnings lost from being injured.

It says you need the entire for you received including proof of service page 6. This I have a question about as you stated your only received one page. Did you receive a proof of service page? If not ask I&A if that matters and if it does call the defense attorney and have him send it to you ASAP. It is a simple form to fill out but you have to upload the form you received if you cannot do that on your computer you can get help at a WCAB. I would like you to go to a district office as they have kiosk there for this. I don't know if that means they still mail you the check or just provide it there. I HOPE you qualify for this benefit. I will be crossing my fingers for you. Again if you need my help let me know and I will try my best.

How to apply:

The application for the one-time $5,000 payment is available online. If you do not have access to the internet, every DWC district office has a kiosk equipped with a computer, scanner, and printer for your use. Information and Assistance Officers are available in person and over the phone.

You will need the following information to complete the application:

The entire SJDB voucher including proof of service (page 6)
SJDB voucher in .pdf or .tiff format for uploading and attaching to online application
Adjudication (ADJ) number
Use the public search function to find your case and ADJ number. If you do not have an ADJ number, please contact the Information and Assistance Office nearest you.
Workers’ Compensation claim number
Claim numbers are issued by the claims administrator when an injury claim form is filed.

Thank you. I would really appreciate if you help my file it. I don’t even know where I will put all my things if they put the eviction sign on my door.

So let’s say that the qme report does say return if light duty is available. I dont think he will give wpi yet because he wants me to do physical therapy and also see a specialist. I remember him saying that he will look into mmi date after physical therapy.
