One of the more interesting aspects of stories liek tis is how it actually happens. A Dr. who at one time worked for an IME mill company in Western NY with multiple site and numerous DRs doing exams all day explained that errors which creep into reports are not intentional as they are so easy to discredit and so would serve no real purporse. He explained that IME Drs. may see man patients in day, a dozen sometimes and it's not unusual to scribble some notes down i theh exxam and then move on to the next exam. The Dr. then sits down at the end of the day to dictate the days exams and utilizes patient intake forms and existing records along with his memory of the patient to formulate a history and exam.

It's not unusual to get one patients statements, such as history of depression, confused with another patients' and this error then becomes part of the report. Unintentional perhaps but no less problamatic until the Dr. can be deposed.
