I also keep records of everything that i do with the injury i substained. I keep one envelope for a attorney if needed and the other for myself. I needed PT/OT records a few months back and went right to medical records and signed a release form. The secretary made copies for me and did not charge me.
Also a few weeks ago, i had my PIR rating. I went up to the doctors office after my PM appt. and asked for a record of the rating. The secretary told me that they just sent it to the adjuster and that the adjuster owns it now and i would have to directly ask them for the copy. I emailed my adjuster to send it alone with the impairment rating award. If it does not come with my rating , i will call and request it again. I have heard of having to pay for copies , like .15 cents a copy, but NEVER 1.00. They must be discouraging people from comming there to get their records like others have said. Maybe if you go in person there and see if they can give you a copy would be your best bet. Has the cost of tree's gone UP? LOL or is it the cost of the INK? I'm in PA. and I have had to pay high amounts for my records. It seems that each Dr. or hospital charges their own price. I have paid $1.25 per page for the first 20 pages and $.75 for the next 20 pages and $.25 for all pages after 40, this was from a hospital. I have also had to pay different amounts at different Dr's. If my lawyer requested the records, their was an additional $18.00 fee from the Dr's. office. I think it is better for your lawyer to request the defence to supply him with the records under discovery terms. Good Luck.

I have all of my tests etc. that are done by different Dr's sent to my PCP. When I go to him I ask for a copy and the girls always make it for free. They are your records and owned by you, but the office can charge for copies. MRI's you can usually get a disk for, but not the hard copy. The IC lawyer just requested a copy of all my records from all of the DR's because they are sending me to a IME. I have already given my lawyer all the copies of all my tests, but they want their own.

I too have never had much luck obtaining my own records. My lawyer has them all...and I have requested them from him....but have not said yes or no, as i figure since there is a massive amout of records (per his statement) it may cost me a bundle to get his office to copy them all for me. Once I paid $10. for a copy of a report at the hospital and they needed a reason as to why I wanted it??!! Tee'd me off, I told her I was exercising my right to my own medical records...but it cost me 10 bucks Ridiculous!

Woah Lily I just sand the release forms and usally I get a copy sent or at next appointment ..
I was once told that they charge and even got a copy of what it would be for 20 forms but the girl never asked for the money ... Not pushing it ... But cant remember what to ask for ... doctor reports . test . intake forms But have been told that clinic doesnt have nurse intake .. but can see the papers each time I go to the clinic that they do indeed make notes .... But I cant look at my total record ..... which I find stupid why not . |
