Hi Bad Boy,

The letter came from my employer via UPS and required a signature to receive it.  

The letter that was sent to me included the name and address of the employer.  I took them out and put (name of employer) and (address) because I didn't want to post it here on the forum.  Call me paranoid.  As for the date fields they were left blank for me to fill in (I assume).

Why they invented their own form is beyond me, but I must assume that their intentions are to obtain information that is not at their access without my authorizatoin, to protect themselves, to pay as little as possible on my claim or to deny my claim entirely.  I cannot foresee a reason they would be requesting this information that would be in my best interest or to benefit me.

However, I am still weary as to the reason they sent this to me instead of requesting the information straight from the IC.  I would almost bet my employer requested information from the IC and they refused to give them anything except what pertained to my workman's comp case and thus the letter sent to me requesting permission.  Worse yet maybe the IC gave them my file and they found something they believe they can use against me but they can't because it was obtained illegally.  There are just too many queazy feelings here so I guess the best option for me is to CMA (cover my a$$).

I think maybe you are right maybe I should just not sign the document at all.  They really should go through the IC instead of me.  Maybe I should just write across the form that it is not a State of Texas Worker's Compensation Form and return it just so that I return something before their May the 14th deadline.

Deb, please remember I said in my Opinion. It also might be wise to consult an Attorney about it, or even Your State WC Board.

Very good post Bad Boy!! I never thought to think that it should of gone to the attorney first. Just goes to show why you are the GODFATHER of the board, and all I have is control over a crazy Penguin!! (LOL) Thanks again for a great post!!Wink

Well, I don't know if they have an Attorney, but they can call the State Ombudsman if they don't. Might as well get them close to best advice, then an opinion or a maybe. Better safe than sorry always....
