Division Of Workers' Compensation
Form EES-14 ( rev. 01/07 )
States the following.

Use the attached form Dwc-69 (rev. 10/05) to report your findings and submit the form and documentation supporting the calculation of the impairment rating no later than 7 days after the examination to the employee, employee's representative if any, treating doctor, insurance carrier, and the Division field office handling the claim.

I am trying to find the rule or statue that supports that statement. The ombudsman service is useless on this one.

AQA? Is that what you got in the mail cause you went for your IR? I am curious as i am waiting now for the adjuster to sign the legal papers for me to set up mine with the doctor.

Sparky, Yes. That was the from the letter I received telling me about the appointment. I am trying to find the law or rule that says it because of the issue I had on the phone with an ombudsman.
