And of course everyone.
Texas Department Of Insurance Division Of Workers' Compensation Form EES-14 ( rev. 01/07 ) States the following. Use the attached form Dwc-69 (rev. 10/05) to report your findings and submit the form and documentation supporting the calculation of the impairment rating no later than 7 days after the examination to the employee, employee's representative if any, treating doctor, insurance carrier, and the Division field office handling the claim. I am trying to find the rule or statue that supports that statement. The ombudsman service is useless on this one. 8-05, Micro laminectomy/disectomy. 10-05 lumbar fusion L5-S1. 2-07 exploritory surgery. 12-07 medical implant, Spinal Cord Stimulator. now receiving SSDI. AQA? Is that what you got in the mail cause you went for your IR? I am curious as i am waiting now for the adjuster to sign the legal papers for me to set up mine with the doctor.

Sparky, Yes. That was the from the letter I received telling me about the appointment. I am trying to find the law or rule that says it because of the issue I had on the phone with an ombudsman.

The complete requirements for filing DWC-69, including required documentation, are covered by Rules 130.1 through 130.6

1171, yes I got it in time. I am filing a complaint against the DD in reference to an administative violation.
