Bronco this is a questionaire (DDS) Disability Determinations Services requires individuals who have filed for SSDI to complete. This helps them get a better picture of your level of function.

My oldest daughter was on SSI for a few years due to her disability, I completed a form similar to this when I first applied for SSI for her.

I'm hoping that after they recieve it they render thier decision.... I have never been one for waiting, almost as bad as the wc waiting games....

We had ours turned in within the month. Still took the 4 months and also had to see a Dr they sent him to.
