I want to thank all those that reply to this thread.
I am surprised to see such short time frames for the most part. I do not want anyone to get discouraged if they don't get a responce from SSA in the same time period as others. To add myself to the mix. I was denied 6 weeks after applying. It took about a week to get more documentation and submitt it for reconcideration. About 6 weeks latter I recieved a check. A few days after that I recieved the approval letter. Total time, 13 weeks. 8-05, Micro laminectomy/disectomy. 10-05 lumbar fusion L5-S1. 2-07 exploritory surgery. 12-07 medical implant, Spinal Cord Stimulator. now receiving SSDI. Jesus died for our sins. Soilders died for our freedom. |

I have heard some horror stories but most were because of lack of documentation.Because of reading on here and help from others on here I was prepared with all my medical paperwork all my Drs names and addresses. I had made an appointment with my PCP and told him what I was going to do and he helped me alot.Being prepared with everything they are going to require is half the battle.
;)Workmans comp is not a road you want to travel alone.You need a good lawyer,a great family and good friends to lean on.If you make it thru without losing everything you have worked for all your life,you have come out ahead of the game.....

It is very helpfull for me as well as others in the need to see this.. I thnk Im about 45 days in the waiting period of decision.. A few days ago I recieved a letter from ssa, needing me to fill out a ssa-3373-bk form and to have it sent back within 10 days or they would rely just off my medical records to make thier decision....
The form is simply lame but its thier way of knowing how I function on a daily basis..... Its called a Adult function report 9 pages And is full of questions and spaces to explain ... I agree Jayne, being prepared goes along way. Everyone should have copies of thier medical records.
Bronco, it looks like what I had to fill out when I applied on-line. Except that is has been modified. 8-05, Micro laminectomy/disectomy. 10-05 lumbar fusion L5-S1. 2-07 exploritory surgery. 12-07 medical implant, Spinal Cord Stimulator. now receiving SSDI. I received this form. I feel it is used to "trip you up". Age is a big factor in determining if you are considered disabled with SSA. Anyone under 50 will have a harder chance. How do we consider age ?
We consider your chronological age in combination with your residual functional capacity, education, and work experience. We will not consider your ability to adjust to other work on the basis of your age alone. In determining the extent to which age affects your ability to adjust to other work, we consider advancing age to be an increasingly limiting factor in your ability to make an adjustment to other work. If you are a younger person (under age 50), we generally do not consider that your age will seriously affect your ability to adjust to other work. However, in some circumstances, we consider that persons aged 45-49 are more limited in their ability to adjust to other work than persons who have not attained age 45. If you are closely approaching advanced age (age 50-54), we will consider that your age along with a severe impairment and limited work experience may seriously affect your ability to adjust to other work. We consider that at advanced age (age 55 or older) age significantly affects your ability to adjust to other work. We have special rules for persons in this category who are closely approaching retirement age (age 60 and above). |

I agree lucky, some of the queations were tricky in answering them.. I can see why though, Im only 42 but personaly when my mind is straight I often think im younger but my body is another story...

We received similar forms. I also had a set to complete. Although our answers we're the same, they we're very different. A quick example is...taking care of the pets. He put yes he feeds them. I put he feeds them as we'll but also put myself or my son get the bag of dog food and put it in a container so it is up off the ground and doesn't have to bend over as far. He Carries bowls to their eating spots. He does not water them. The water bowl is too heavy when filled. I fill before I leave for work. My son will refill if needed before I get home. A gallon of bottled water is kept next to bowl incase of 'emergency' he can use it but has never had to yet.
His questions had several my wife 'encourages' me answers. I had several I do it all. Lol. The frustration was obvious in both our answers. |

I like the one where it asks what do you do from the time you get up until the time you go to bed. Come on really....... I dont kn ow about yous but every day seems to be a new chalange wondering when the pain is going to take yyou down. As far as the pets go, i let them out but as far as feeding and baths grooming is the wifes duty..
The only question I worry about is do you take care of anyone? It bothered me to answer it.. But if they did a visit they would see how we have Dominicks room set up, crib is raised and so is the bed in there I use to change him when needed.... I also stated I only watch him for a few hours in the morning until my wife comes home.. As I can see, you all prety much have had to fill this out... What I'm wondering is, did you fill this out when you first applied...? And if this came later, how long before they made thier decision after you sent this form in..? |

Bronco I filled it out on line ...when I originally applied for SSDI....
Cervical Fusion 2003, c5-c6. Herniated and damaged Disc L1- L4-L5 S1. Lumbar Spinal Cord stimulator implant 09-2008. Cervical ACDF revision with hardware c4-c5-c6-c7 Sept 2009. SSDI approved 3-2010. NOW OFFICIALY RETIRED |
