Help? I dont know what to do

I tore my rotator cuff while working in 2007. I reported the accident to my company and was sent to a special doctor for work comp. I saw the doctor 3 times and did a few round of physical therapy. The doctor then told me I would need to see a sports surgery specialist to help with it. It would take 6-8 months to get my company to approved and get the appointment scheduled. I never here from them. I call again spring 2009. They dont know why company never answered with an approval. In the mean time ever time I use my work ladder it gets strained and starts popping and hurting again. I call the docotor to ask again in Feb 2010. They said to wait while they get approval from company to come back and see them again. I just recently hurt it again taking ladder off of truck and said I wanted to call company and asked them for contact information. Come to find out they are not calling the company they are calling an insurance company named broadspire. I have never even heard of them. I call them and after talking to them I am contacted back and told since they last paid the docotor in 2008 it has been to long for them to pay. They would have needed to have been contacted sooner. The doctors office contacted them many times and forwarded info. After forwarding info the insurance company never answered them. The only response the docotor received was a request to close the case in 2008. What do I do? I am still in pain and been left waiting for years! Had I known there was an insurance company to contact I would have. They never contacted or informed me of anything.
