Form 36s
hi people, i'm CT and receive a letter from the state and i don't know what it mean,i been in wc for almost 2 years now,the form name is 36s,any help please,tnx.

In CT, the 36 form is a "Notification of Intention to Reduce or Discontinue Benefits".
This is filed with the CT Workers Compensation Commission when the ER/IC (Employer/Insurance Company) intends to reduce your weekly benefits or stop them all together. You have 15 days from the receipt of that form to appeal and ask for a hearing. DISCLAIMER: I am not an attorney. if you appeal you'll be able to hear their evidence at the hearing and you can present yours.

Remember, though, you have 15 days from the date of receipt of the Form 36 to file an appeal. You can call one of the numbers on the bottom of the form to arrange an appeal hearing - DON'T WAIT.
