Can an employer deny a prescription pain killer when you have already told the doctor that OTC pain killers were not working?

yes.
it depends on the reason for the denial and the state.
many states have utilization review systems in place that restrict treatment (including Rx) for certain conditions.
class 2 drugs are often restricted to specific situations.

have your doctor fax a letter to your workcomp adjuster explaining why the medication is needed. sometimes thats all it takes
