RE: Have a Question
they can ask; it's up to the doctor.
you should consider a second medical opinion if you are not getting better.

RE: Have a Question
contact your claims adjuster directly and ask for a list of network doctors to pick from.
follow up by also sending your request in writing.
you can also go here to find one

RE: Have a Question
If you are being seen by this doctor, it would simply be so much easier to obtain a copy from the employer, of the new up-dated form-73, and show it to your doctor, and ask the doctor all about it. Then you can ask, how it is possible it states you were there when you weren't, and you didn't sign the form. This mostly would get you an answer way quicker.

RE: Have a Question
Plus the fact, many things lay in your hands also. If you continue in pain, and it appears to become worse, you need to explain this to the doctor right away, for a new evaluation on your condition. Then the doctor is allowed to make more changes. But, if pain increases, you need to inform the employer, I would rather do this in writting, ask for you to sign and date, and employer sign and date, then obtain a copy of it. Then go somewhere and make more copies of it, so you have extras when needed. Then at time of advising employer of the increases in pain, demand they send you to the doctor, with a new company work slip to see such doctor. You can do the same thing right back, just do it, as it is meant to be done properly.
