automatic penalties are paid with the benefit check to the injured worker.
a penalty filing requires a decision by the judge and is payable as are all awards within 20 days barring an appeal.
should a C&R be negotiated to include penalties they would be part of the C&R amount. if there is no language covering the penalty and mileage issues then it's not settled and still outstanding.
it's unusual to have your atty prepare the documents. "performing" is not the same as preparing.
it is required to have a DOR before a hearing date can be set.

yup. with limited time for your case she can either answer your questions or work on the C&R but probably not both.
