why don't you discuss it with your employer? if the employer has any doubts they can request a fitness for duty evaluation. you could also take a job description to the doctor at the time of the rating and have them ok your duties

yes, treatment for the injury is covered. as with anything there could be a dispute about it's relationship to the injury.
temporary disability is limited to 104 weeks within 5 yrs of the date of injury.
aggravations are considered a new injury and you can file another claim.
most other insurance excludes workplace injuries unless it's denied by workers comp so yes they would cover it with a work comp denial.
