Each state has different laws. those references don't mean much in the other 49.
if you want help let us know what state you are refering to.

I didn't find much with those reference numbers:
http://www.wcb.state.ny.us/content/main/...ewlaws.jsp
I couldn't find a form that might help:
http://www.wcb.state.ny.us/content/main/...AIMANT.jsp

it sounds like a cover sheet for submitting updated evidence.
if you don't have an atty
I'd call the advocate for injured workers for help:
Advocate for Injured Workers
for questions related to injured workers:
(518) 474-8182
(800) 580-6665

or the NY work comp board service center
NYS Workers' Compensation Board
20 Park Street
Albany, NY 12207
Customer Service Toll–Free Number
(877) 632-4996

or send them an email

now that we have the form number it appears that the employer/carrier is requesting the comp board change some of your benefits.
section b of the form should tell you what exactly they want the board to decide.
It would be illegal for them to ignore your claim as long as they are required to make payments, pay benefits and adjust the claim.
