I Live in Kentucky ....I just called a lawyer and they said the doctor would have to state that it was due from my type of work, and that if you received the ok it was due from work, I need to go to the HR dept. and asked to file a claim for workman's comp sooner than later.
When you filled on line what site did you use ? I may just do that instead, it would not hurt. Please let me know how do go about that

As of now the company is using an affiliate of my insurance company to pay me short term disability.Therapy expenses.

Also, on the left hand side, click "injured worker" and see if there is anything that will help you.

I looked up that and I am almost scared to fill it out without an attorney, although I could. I am going to see if the doctor calls me back Monday, and if not I will make an appointment to see her. I do not have a Doctors statement other than when she filled out my FMLA papers stating I could be off of work for up to 3 months beginning 8/17/2017. I know my doctors knows this is caused from my work, but she may be in with the people at the plant, because my boss and others see her from my work....Who knows...
Thank you, and I have saved this form 101 and also the web site For future use... I need all the advise I can get. I live alone and pay a mortgage, I am afraid of losing everything. Depression and anxiety setting in.. Thanks for the info.

Since it's not yet a comp claim you can go to any doctor you choose.

Yes many of us had to deal with more severe problems than you are having. Ive been reading and reading and  you seem to refuse to say what your state is where this happened  If you want accurate advice knowing your state would make that possible.

Just call the comp office ? I will do that ASAP I have the number for Kentucky now. thanks
