I have an Hearing with the Industrial Commission next week and the C-256 (Employer Notice & Request for Information Form) was located under the Notes Section on BWC website, however it was not listed in the correspondence section. It said something about 14 day diary, but that was all the information provided. I checked all the forms on the BWC website and the C-256 form was not listed. What could it be?
