Full Version: Mileage Reimbursement for NY

I injured my lower back on September 24, 2005. It took a couple of months to get all the paperwork going, but since then I've hardly had any problems with payments, dr visits, and/or prescriptions. The only mistake I've done was since the date I injured myself I never filled out any forms for my mileage. I just assumed the lawyers I got were handling that. Is it possible for me to still be reimbursed going all the way back to 2005? They have all my visits on file and since then I've seen a handful of specialists, had procedures done, and travelled from one part of Long Island to the other. If I'm not able to go back that far then how far back can I go in which I can be reimbursed for mileage? Imagine if there was someway for me to get all of that from 2005..If someone knows of a way for this to work and I actually get reimbursed from 2005 til now you will not be forgotten. Thank you
you can ask for an itemization of your treatment and then calculate your mileage request. put it in writing to your carrier.
So all I do is call up my claims adjuster or anyone from my carrier and just ask for an itemization of my treatment? I take it that's a list of all the dr. and chiro appts, as well as any procedures I had done? Well, if that's the case and all I need to do hopefully it works out. Not all states pay for mileage for regular treatment. Some only pay for employer selected IMEs. In any case, you need to apply on form C257 "CLAIMANT'S RECORD OF MEDICAL AND TRAVEL EXPENSES"
Be sure to copy in your lawyer so you have proof of the request.
