I'm looking for a statute reference (or adminstrative rule) for the posting requirements for the state of Iowa. Can anyone help?

Such employer shall post and keep posted in some conspicuous place upon the premises where the business is conducted, a notice in form approved by the workers' compensation commissioner, stating the nature of the security furnished by such employer to secure the compensation payments contemplated by the law. "
But I can't find a form anywhere for the notice referenced. I've searched the IWC website, the Iowa Code, & Adminstrative Rules. Anybody have a clue whether a standard form for this exits? (I've received such forms from our carrier for CA employees, but never for IA employees).
Thanks BBBB-but the poster doesn't list a form for W/C info. The state's web site has a listing for required postings, but a W/C posting is not listed there. I'm beginning to find it hard to believe that this would be a universal requirement and yet I can't find a sample or template anywhere!

Go to the bottom of the site, click your State, then read to your right, it list Workmens Compensation. These are the Posters many are using now, do to there all in one, in your case all in 2 Posters, to save wall space.
