Is there a form available to wave employer liability when employee is working from home?

generally workers comp coverage is mandatory and can't be waived. some states allow exceptions or exlusions for certain classes of workers but technically that's not a waiver. Coverage requirements are determined by each states comp laws; there is no one answer.

Where can I find documentation to see if Wisconsin has exceptions?

According to workcompresearch.com, workers' compensation is compulsory in Wisconsin, waivers are not permitted.
definition of covered employees does not exclude those who work from home
